This session will discuss a team-based approach to diabetes management including workflow, best practices, and billing.

Learning Objectives for pharmacists

• Discuss assessment of patient cases for appropriate diabetes management and care.
• Identify key interventions that will make a difference for patients with diabetes.
• Review new products approved for Type 2 Diabetes.
• Summarize programs available to pharmacists such as DPP and DSMES.
• Discuss workflow strategies for the entire pharmacy team to provide diabetes care to patients.
• Discuss assessment of patient cases for appropriate diabetes management and care.

At the completion of this activity, the participant will be able to:

• List new medications used by Type 2 Diabetic patients including the generic and brand names, strength, dosage forms and routes of administration
• Review steps to assist the pharmacist with record keeping and documentation for diabetes medical care.
• Discuss workflow strategies that assist the pharmacy team with diabetes medical care.
